Theo số liệu khảo sát, cứ mỗi 10 người thì có 8 người từng bị đau lưng ít nhất một vài lần. Những cơn đau lưng có thể ngắn hạn hoặc đau dai dẳng này gây khó chịu, bất tiện trong sinh hoạt, thậm chí là dấu hiệu của nhiều bệnh lý nghiêm trọng cần được phát hiện sớm.
1. Nguồn cơn của cơn đau lưng lan xuống vùng lưng và mông
Có rất nhiều bệnh lý về xương khớp gây ra cơn đau lưng lan xuống mông và chân
1.1 Đau thần kinh tọa
Đau thần kinh tọa không chỉ do thoát vị đĩa đệm mà còn nguyên nhân khác gây ra như hẹp ống sống, thoái hóa cột sống, trượt đốt sống hoặc do làm việc sai tư thế, thói quen xấu, ngồi lâu, ít vận động,… Cơn đau chạy dọc theo đường đi của dây thần kinh tọa, bắt đầu từ đau tại cột sống thắt lưng, sau đó lan đến hông, mông, mặt sau của đùi và các ngón chân, kèm theo biểu hiện nóng rát kể cả khi nghỉ ngơi.
1.2 Thoái hóa cột sống
Đây là một căn bệnh xương khớp mãn tính phổ biến nhất ở người cao tuổi, nó bắt đầu khi các khớp cột sống và đĩa đệm đã bị thoái hóa, bào mòn theo năm tháng. Giai đoạn đầu của căn bệnh sẽ tạo nên các cơn đau thắt lưng âm ỉ, kéo dài và tăng lên khi người bệnh vận động, di chuyển, cúi người, xoay trở mình hay mang vác các vật nặng. Giai đoạn nghiêm trọng là lúc các cơn đau ở lưng lan xuống chân, gây tê liệt và mất thăng bằng khi di chuyển.
1.3 Thoát vị đĩa đệm
Triệu chứng cơn đau lưng xảy ra khi phần nhân nhầy của đĩa đệm bị lệch ra khỏi vị trí bên ngoài, tạo sự chèn ép lên dây thần kinh ở cột sống. Biển hiện là các đơn đau dữ dội, tê bì kéo dài ngay tại vùng bị ảnh hưởng, trong trường hợp dây thần kinh bị chèn ép quá mức thì có thể bị đau lưng lan xuống mông và chân.
1.4 Gai cột sống
Gai cột sống gây ra triệu chứng đau tập trung ở thắt lưng rồi lan xuống phần mông và chân, mức độ cơn đau sẽ tăng lên khi cơ thể vận động như cúi người, mang vật nặng, xoay trở người.
1.5 Hẹp ống sống
Là biến chứng do thoát vị đĩa đệm gây ra, xuất hiện ở nhiều vị trí khác nhau trên xương sống, nhưng phổ biến và nguy hiểm nhất đó là hẹp ống sống thắt lưng. Bệnh nhân sẽ phải đối mặt với các triệu chứng từ nhẹ đến nghiêm trọng như đau lưng dữ dội, tê bì hai chân và mông, đôi khi còn gây ra hiện tượng co thắt cơ và yếu cơ.
1.6 Chấn thương
Di chứng từ chấn thương thể thao, trong lao động làm việc hoặc tai nạn giao thông gây ra đã làm tổn hại đến phần cột sống nên gây ra các cơn đau lưng hoặc có thể kéo dài xuống thắt lưng rồi cuối cùng là các đầu ngón chân.
1.7 Cột sống lưng bị nhiễm trùng
Nguyên nhân làm nhiễm trùng cột sống lưng là hậu quả của bệnh ung thư, bệnh tiểu đường hoặc di chứng không mong muốn của phẫu thuật gây sưng, viêm và đau thắt lưng.
1.8 Viêm cột sống dính khớp
Viêm dính khớp cột sống gây ra các cơn đau xuất phát từ vùng xương chậu sau đó lan dần sang các vị trí xung quanh, trường hợp nặng có thể gây đau cả vùng cột sống và lan xuống mông, đùi và hai chân.
1.9 Bệnh lý khác
– Bệnh phụ khoa: Đau lưng dưới ở phụ nữ đi kèm tình trạng ra khí hư nhiều, vùng kín có mùi, chảy máu âm đạo… là dấu hiệu của các bệnh phụ khoa (như viêm âm đạo, viêm cổ tử cung, ung thư buồng trứng).
– Bệnh suy giảm chứng năng thận: Khiến vị trí thắt lưng bị đau nhức, bệnh nhân di chuyển khó khăn, không thể cúi người, quỳ gối được như bình thường.
2. Nếu không điều trị kịp thời thì sẽ gây ra nhiều biến chứng khó lường
Căn bệnh đau lưng nếu không sớm điều trị và đúng cách thì sẽ để lại nhiều chiến chứng nghiêm trọng cho bệnh nhân, bao gồm:
– Làm suy giảm khả năng vận động, khiến sinh hoạt hằng ngày của người bệnh rất khó khăn, tệ hơn có thể làm teo cơ, liệt chi dưới.
– Phải chịu đựng những cơn đau “thấu trời” kéo dài liên tục, khiến bệnh nhân mất ăn mất ngủ, chất lượng cuộc sống từ đó mà cũng giảm đi.
– Trong trường hợp bệnh kéo dài sẽ dẫn trở thành mãn tính, khó điều trị làm tốn kém chi phí, công sức lẫn thời gian.
– Nếu biến chứng nghiêm trọng thì có thể khiến cột sống cong vẹo, lệch, làm biến dạng cơ thể và làm gây tổn hại đến các bộ phận khác bên trong cơ thể như gan, tim, dạ dày, đại tràng,…
Ngay khi xuất hiện triệu chứng đau lưng xuống mông và chân thì người bệnh nên đi gặp bác sĩ để được khám và chữa kịp thời nếu không muốn để lại những di chứng và hậu quả khôn lường.
3. Các phương pháp khám và chẩn đoán
Để xác định chính xác nguyên nhân gây ra triệu chứng đau lưng, thì bác sĩ sẽ khám lâm sàng và tìm hiểu về tiền sử bệnh lý của bệnh nhân, các triệu chứng diễn ra như thế nào, mức độ và tần suất của cơn đau. Một số xét nghiệm sẽ được tiến hành nhằm ra kết luận chuẩn xác nhất:
– chụp phim X-Quang.
– Điện cơ
– Chụp ảnh CT hoặc kết hợp chụp từ MRI
– Xét nghiệm máu trong trường hợp bệnh nhân bị nhiễm trùng dẫn đến sốt cao hay sụt giảm cân nặng không rõ lý do.
4. Các biện pháp điều trị phổ biến nhất
4.1 Dùng thuốc giảm đau
Đây là phương pháp đầu tiên và đơn giản nhất mà bệnh nhân có thể nghĩ đến bởi vì nó mang lại công dụng nhanh chóng và hiệu quả. Tuy nhiên, sử dụng các loại thuốc giảm đau, thuốc giãn cơ và thuốc chống viêm chỉ là biện pháp tạm thời và nếu sử dụng trong một thời gian dài hoặc tự ý gia tăng liều lượng mà không có sự chỉ định của bác sĩ sẽ dẫn đến các tác dụng phụ vô cùng nguy hiểm.
4.2 Phẫu thuật cột sống.
Đây là giải pháp cuối cùng khi người bệnh không còn đáp ứng được các biện pháp chữa trị nội khoa hay triệu chứng đã trở nên quá nặng có thể gây nguy hiểm đến tính mạng. Tuy nhiên, rủi ro sau khi phẫu thuật cũng vô cùng lớn nên bác sĩ và bệnh nhân sẽ luôn cân nhắc và xem xét kỹ lưỡng để đưa ra quyết định sáng suốt nhất.
4.3 Vật lý trị liệu – phục hồi chức năng là phương pháp bảo tồn xương sống một cách an toàn, hiệu quả nhất
